MATRIX MODEL

Ordinarily the planning and construction of a MATRIX outpatient wound and hyperbaric center will take at least 16 weeks for completion. UHMS accreditation standards are utilized at the very beginning of the planning stages and throughout the entire build out to maintain safety compliance..

The MATRIX Model utilizes a Medical Director to oversee a team of physicians (preferably about 6-8 physicians of varying specialties) to allow physician coverage for at least forty hours a week of clinic time, as well as, emergency coverage and in-patient consultations as needed. Ideally physician team members will be of varying specialties to provide patients a multi-specialty approach to their particular disease process needs, and also to ensure that all specialty groups are informed of the benefits of referring patients to the wound care program.

Physicians act as independent practitioners. Each Physician will cover a specified time (usually 4-8 hour block) overseeing wound care and hyperbaric oxygen therapy. MATRIX will provide tools to assist physicians with billing processes. Superbills that can be used for hospital billing of technical fees and physician billing will be provided.

The Medical Director is generally paid an hourly stipend for duties such as physician team scheduling, performance improvement and quality oversight, and providing training and education to other clinicians. These activities generally require 10 to 20 hours per month. The hospital can make payment to the Medical Director directly, or MATRIX can pay the Medical Director and pass through costs.

The Program Director is ultimately responsible for the outpatient program's clinical, operational, and financial performance. The ideal Program Director candidate will have clinical knowledge of wound care and have a strong background in management or business administration. A registered nurse with an advanced degree such as a MSN, MHA or MBA would be a good candidate.

A Clinical Coordinator oversees clinical practice of the day to day unit activities. This position must be filled by a registered nurse.

The wound care staff is a multidisciplinary team of physicians, nurses, physical therapists, and technicians. While one Registered Nurse can case manage as many as 50 outpatients, a staffing matrix based on daily stats will be used to determine numbers and levels of staff needed for a particular program.

The Community Education Liaison is responsible for educating the referring physicians, healthcare workers such as case managers and discharge planners, and the general community in the value of referring patients to the wound center when wound care is needed. This role will require excellent communication skills as well as a thorough knowledge of wounds and wound healing techniques.

Hospital Administrators and MATRIX will determine the most appropriate business model for the Business Agreement. There are a multitude of options for the business model, even under a management arrangement. Options depend on the amount of risk and reward the administration would be willing to take. If Administration would like to pay for all build out, equipment, and operational expenses, MATRIX can quote a flat monthly rate. However, it may be added assurance for administrators to choose a business model where MATRIX is given a conservative monthly rate, with a volume incentive.



Once the business model is determined, MATRIX will follow a very detailed and comprehensive "Start Up" plan. This plan covers oversight of the design and build out, budgeting, selection of equipment, recruiting of MD team, hiring staff, providing policies and procedures and protocols, facilitating education of staff and MDs, developing marketing plan, facilitating the set up of an outcomes monitoring system, and providing coding, billing and reimbursement processes and support.

During project development MATRIX will meet with all hospital department directors, in one-on-one sessions or in small groups, to discuss the new service line of a comprehensive wound care program. We feel that it is important to meet with the directors in the initial planning stages to achieve maximum program "buy in" and achieve the best interdepartmental efficiencies possible. The Wound Care Center will become an integral part of your hospital and will blend seamlessly with other hospital departments.

Prior to instituting the program, all physicians and staff will be trained in wound care and hyperbaric oxygen therapy, and will also be educated on appropriate wound care and HBOT protocols, documentation, outcomes trending, and performance improvement processes. The Program Director should be the first employee hired and will help to organize the program, well before the outpatient center is opened. The Program Director will maintain daily communication with MATRIX through on site visits, phone call availability (24/7) and emails.

MATRIX utilizes a web based program to monitor and trend a wide range of clinical and operational indicators. Reports tracking productivity, clinical outcomes, ancillary services generation, referral trending and profitability will be submitted to administration quarterly and as needed. Our program benchmarks clinical outcomes with as many as 1500 users across the country.
